Teacher Recommendation Request

During the year many students will need staff recommendations for special programs, awards, scholarships, colleges, and employment. In order to process requests promptly, students are asked to complete a Recommendation Information Form.  This form provides the instructor with additional information for the instructor to use to personalize their recommendation letter. 

If requesting a recommendation, the student must first ask the instructor to ensure that he or she feels comfortable writing a recommendation. If so, the student will complete the Recommendation Information form and provide the instructor with information on deadlines and where to submit the letter of recommendation.  Students should check back with the faculty member to ensure the recommendation has been completed by the deadline.

It takes time for a faculty member to write a strong recommendation. Therefore, a minimum of two weeks between the time the form is submitted and the time the recommendation must be mailed is required.
